Focus Physical Therapy offers a Wellness Package
option for patients who pay privately, in advance, for office visits. This gives our patients more options to continue to get stronger after finishing their regular visits. Our Wellness Packages also allow more patients to experience our services even if they are out of network or do not have insurance.
6-visit pre-payment $450 = $75 per visit
8-visit pre-payment $560 = $70 per visit
12-visit pre-payment $780 = $65 per visit
15-visit pre-payment $900 = $60 per visit
Traction tables offer decompression of the spinal segments that allow for relief of the surrounding neural and muscular tissues. Traction can aide in improving range of motion and mobility of tissues as well as reduce irritation of symptoms.

Most insurances are accepted. Please call to inquire with further questions at 920-893-6070 or talk to your insurance directly. Our Wellness Package is another excellent alternative for those who are not able to get insurance coverage as well.
Some insurances, such as Medicare, DO require a doctor's referral for services to be covered. Other insurances, do not require a doctor's referral. Please ask us for more details.
At Focus Physical Therapy, we offer Wellness Packages. These packages allow patients to buy a bundled amount of Physical Therapy sessions to address their rehabilitation needs.
We verify your insurance on your first visit and will have your specific information ready for you at that time. The Wellness Packages can be very helpful if you have a high deductible as well as if you want to continue your care after insurance ends.
You should arrive early to complete your paperwork. Then your Physical Therapist will take you back to an individual treatment room to gather a subjective history, complete objective tests and measures and end the session with a plan for treatment.
This really depends on each patient's individual presentation. Your Physical Therapist will go over this after evaluating you on your first visit.
