OUR PACKAGES

Wellness Packages

Focus Physical Therapy offers a Wellness Package option for patients who pay privately, in advance, for office visits. This gives our patients more options to continue to get stronger after finishing their regular visits. Our Wellness Packages also allow more patients to experience our services even if they are out of network or do not have insurance.

Treatment Packages:

*6-visit pre-payment $450 = $75 per visit

*8-visit pre-payment $560 = $70 per visit

*12-visit pre-payment $780 = $65 per visit

*15-visit pre-payment $900 = $60 per visit

Traction Packages

Traction tables offer decompression of the spinal segments that allow for relief of the surrounding neural and muscular tissues. Traction can aide in improving range of motion and mobility of tissues as well as reduce irritation of symptoms.

Peggy Huhn, MPT, Founder

Peggy Huhn is the owner at Focus Physical Therapy and she is the visionary of our small business. She finished her BS Degree in Biology from Marquette University in 1997 and continued on to complete her Masters Degree in Physical Therapy from Marquette two years later in 1999. She lives in the Plymouth area with her husband and 4 children. She has a passion for helping others with shoulder pain, foot pain, gait analysis and adolescent sports enhancement.

Carolanne Bilitz, PTA

Carolanne Bilitz received her Physical Therapist Assistant degree from Milwaukee Area Technical College in 2000. She currently resides in the Oakfield area with her husband. She has an interest in helping others with knee pain/rehabilitation, shoulder pain/rehabilitation and general orthopedics.

Emily Caruso, Clinic Director, DPT

Emily Caruso is our Clinic Manager as well as a Physical Therapist here at Focus Physical Therapy. Emily finished her BS Degree in Exercise Science from Carroll University in 2013. She continued on to receive her Doctorate in Physical Therapy from Carroll University in 2015. She currently resides the Plymouth area with her husband and child. She excels at helping others with knee pain, general orthopedics and geriatrics.

Paula Heller, DPT

Paula Heller completed her BD degree in Therapeutic Recreation from the University of Wisconsin LaCrosse in 2000. She proceeded to receive her Masters Degree in Physical Therapy from Concordia University in 2004 followed by her Doctorate of Physical Therapy from Concordia University in 2006. She resides in the Kiel area with her husband and 2 children. Her areas of interest include cervical/lumber pain, general orthopedics, geriatrics and adolescents

Leah Ruh

Leah Ruh is our Office Manager and the first friendly face you will meet as your walk into the door! She lives in the Plymouth area with her husband as their six children are now grown. She loves spending time with her horses, camping, and organizing everything! if you have any questions, she is always ready to help!

Samantha Kaufman, DPT

Samantha Kaufman is a Physical Therapist who completed her Athletic Training Degree at Concordia University of Wisconsin in 2011. She then continued on to complete her Doctorate in Physical Therapy from University of Wisconsin-La Crosse in 2014. She currently resides in Glenbeulah, WI with her husband and 2 children. She enjoys helping others with lower extremity pain, running mechanics and overall body mechanics education.

Annie Peters, Collections Manager

Annie Peters is our Collections Manager at Focus Physical Therapy. Annie is married with 2 wonderful children and lives currently in Kewaskum. She has an x-ray background, but has done a lot of different positions in the medical field! She is obsessed with coffee, loves cooking and wine! Her hobbies include decorating/crafting and long walks through Hobby Lobby!

Brenda McBain, Public Relations Coordinator

Brenda McBain takes care of our Public Relations for us here at Focus Physical Therapy and is a Physical Therapy Aide. She graduated in 2004 from University of Wisconsin Oshkosh with a BA Degree in Exercise Science. She has been a Certified Personal Trainer and Group Fitness Instructor since 2006. She has a certification in Nutrition Coaching (PN1) and spent 10-years in the Wisconsin Army National Guard. She currently resides in Plymouth with her husband and 3 children. She loves hiking, traveling, music, family time, running and teaching group fitness classes.

FREQUENTLY ASKED QUESTIONS

What insurances are accepted at Focus Physical Therapy?

Most insurances are accepted. Please call to inquire with further questions at 920-893-6070 or talk to your insurance directly. Our Wellness Package is another excellent alternative for those who are not able to get insurance coverage as well.

Do I need a doctor's order to attend therapy?

Some insurances, such as Medicare, DO require a doctor's referral for services to be covered. Other insurances, do not require a doctor's referral. Please ask us for more details.

What if I don't have a doctor's order, but I would like to be seen at Focus Physical Therapy?

At Focus Physical Therapy, we offer Wellness Packages. These packages allow patients to buy a bundled amount of Physical Therapy sessions to address their rehabilitation needs.

What does my insurance cover? Do I have a copay?

We verify your insurance on your first visit and will have your specific information ready for you at that time. The Wellness Packages can be very helpful if you have a high deductible as well as if you want to continue your care after insurance ends.

What should I expect at my first visit?

You should arrive early to complete your paperwork. Then your Physical Therapist will take you back to an individual treatment room to gather a subjective history, complete objective tests and measures and end the session with a plan for treatment.

How long will I need to be in therapy?

This really depends on each patient's individual presentation. Your Physical Therapist will go over this after evaluating you on your first visit.
